To accomplish this task we will work closely with you to maximize your insurance benefits for all covered procedures. If you have any problems or questions, our staff is willing and able to assist you. The insurance industry has become a highly complex area of health care that has the potential to create a great deal of confusion among patients and healthcare providers. We are providing this information in an effort to inform our patients of the financial policies that regulate our office. There are many misconceptions associated with today’s health care coverage. It is our goal to make every effort to identify these questions early on. We hope this will make your treatment experience in our office as pleasant, efficient and stress free as possible. We are here to serve and assist you throughout your course of treatment.

Insurance


Drs. Barbieri, Colameo, Berardo and Wuebbels actively participate with a wide range of medical and dental insurance companies in an effort to make quality health care more affordable to our patients. Each and every insurance carrier has set forth their own set of very specific guidelines that must be followed in order to insure coverage of the services that we provide. We have gone to great lengths to insure that our staff is well trained and aware of the many distinct policies of each particular insurance plan. It is equally important that you make every effort to understand your particular plan and the rules and regulations that apply to you and your policy.

The goal of most healthcare insurance policies is to provide some form of financial assistance for defined medical and surgical services. The services selected are based on the cost of the policy to your employer and the negotiated arrangements made with the insurance company. It remains the patients’ ultimate responsibility to understand the limitation of their own policy.

Our office has requested certain information on our health and information forms. This information is important to your treatment as well as your potential insurance benefits. We request that you fill these forms out as accurately as possible and if you should have any questions, do not hesitate to ask our staff for assistance. We cannot be responsible for problems that may arise as the result of inaccuracies in the information that you provide. In order to take full advantage of your covered services, it is critical that all insurance guidelines are followed exactly as described by your specific plan. This pertains primarily to properly completed referral forms and up to date eligibility cards. Our office will be happy to offer assistance, however it remains the patients’ responsibility to be sure all paperwork is in order prior to treatment.

For most visits our office will require a potentially refundable deposit that will be applied to charges for uncovered services, co-insurance, as well as insurance deductibles (as defined by your specific plan). For your convenience the deposit can be addressed with a cash payment, bank check, personal check or most major credit cards. You should be assured that when our office receives an insurance payment pertaining to a specific office visit with an explanation of benefits (EOB), the appropriate funds would be refunded immediately.

If you are unable to leave a deposit for planned services, we will gladly submit your treatment plan details to your primary insurance carrier for a pre-determination of your benefits. This procedure will take an undetermined amount of time (2 to 8 weeks, this will vary greatly from carrier to carrier) over which we have very little control. Once approval is received in writing and all prior balances are addressed you will be contacted and final treatment arrangements can be made.
